Care Coordinator II BCForward is looking for Care Coordinator- Remote FL Job Title: Care Coordinator II Location: Remote This position is remote within Florida, but in-person meetings may be required quarterly. If remote work is disrupted, the candidate must be able to work from the office location. Position Type: Contract Shift Timings: Schedule: Monday - Friday, 9:00 a.m. - 6:00 p.m. EST. Duration: 03+ Months Extension: Yes Pay Rate: $25.00 on W2 Position Purpose: Supports care management activities and team coordination to ensure services are delivered by healthcare providers. Engages with members via phone to ensure continuity of care and achieve member satisfaction. Key Responsibilities: Conduct outbound quarterly calls to members to follow up on care plans. Perform research, such as identifying member information and connecting with providers. Make 3 attempts per member (Day 1, Day 2, Day 3) and connect with the PCP if contact is unsuccessful. Handle 50-75 calls per day. Submit a daily report of completed tasks. Work remotely but may need to meet in person quarterly, with the ability to work from the office if remote access is disrupted. Candidate Requirements: Education: High School diploma or GED required. Experience: 2+ years in healthcare/managed care background, experience with high-volume calling. Skills: General technology experience, including Excel, Outlook, Word, and Teams. Preferred: Bilingual (Spanish, Creole). Previous work-from-home experience. Performance Indicators: Ability to multi-task, make calls, and take notes simultaneously. Ability to maintain professionalism while working remotely. Ability to manage confidential medical information.
