The Team Coordinator plays a crucial role in managing daily operations in a hospice and palliative care setting, focusing on organizing clinician visits, office inventory, and interdisciplinary meetings. We are looking for a team coordinator ready to lead and serve. As a team coordinator, you will be responsible for answering all incoming calls, processing daily workflow, assignment of clinician visits, preparing interdisciplinary meetings, managing and maintaining office inventory and medical supplies and supporting the clinical leader(s). Most importantly, you will be responsible for ensuring the smooth operation of the agency so patients can receive the care they need.

Qualifications:
  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Associate degree preferred
  • Experience: Minimum of 1-year experience in a health care setting. Scheduling and office manager experience preferred
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs
